paresthesias, ataxia and bilateral steppage in a young patient

Presentation

A 20-year-old patient was hospitalized 5 days ago following paresthesias of the lower limbs, ataxia and bilateral steppage progressively evolving for about ten days. He has no known medical or psychiatric history.

During this hospitalization, the team carried out various biological assessments (including lumbar and toxicological puncture) and imaging (including cerebral and medullary MRI) which proved to be normal apart from a slight decrease in vitamin B12 to 170 ng/L (normal between 195–770). The skin examination found a burn of the left thigh.

At the end of the hospitalization, the disorders persist but at a minimum, and the evolution seems semi-favourable. In this context, the neurology team concluded that there was a disorder with functional neurological symptoms and ruled out a neurological origin. The patient is therefore referred to a liaison psychiatrist.
