Healthcare Fraud: Indiana Woman Sentenced to Prison

A Highland, Indiana physician has been sentenced to nearly eight years in federal prison after pleading guilty to healthcare fraud. Bethany A. Cataldi, 54, was sentenced on Wednesday, February 18, 2026, to 97 months imprisonment, followed by one year of supervised release, for billing approximately $22 million for procedures that were never performed. The case highlights a significant instance of healthcare fraud and the government’s commitment to prosecuting such offenses.

Cataldi, a former otolaryngologist, admitted to submitting fraudulent claims to Medicare and various insurance companies, primarily for balloon sinuplasty procedures – an outpatient treatment for chronic sinus infections. The scheme, which ran from March 2015 to December 2023, involved scheduling fake surgeries at the Center for Otolaryngology and Facial Plastic Surgery, her practice. Investigators found that at least eight patients confirmed to authorities that their scheduled operations never actually took place. This case underscores the vulnerabilities within the healthcare system and the potential for abuse.

Details of the Fraudulent Scheme

Court records indicate that Cataldi’s practice billed the government for over 1,800 balloon sinuplasty procedures. However, investigators estimated that she only possessed supplies sufficient to perform approximately 100 of these procedures. The total amount billed reached $22 million, with $16.5 million submitted to Medicaid and $9 million reimbursed for the fraudulent procedures, according to court filings. The fraudulent claims were particularly concentrated from December 2017 onward, with approximately 86% occurring during that period.

As part of her plea agreement, Cataldi is required to pay $19,138,245.43 in restitution, as announced by U.S. Attorney Adam L. Mildred. She has also agreed to forfeit significant assets, including jewelry, multiple Mercedes-Benz vehicles, musical instruments, and funds from several bank accounts. Cataldi will sell real estate and other luxury assets acquired with the proceeds of the fraud to fulfill her financial obligations to repay the fraudulently obtained funds, according to the U.S. Attorney’s Office, Northern District of Indiana.

Legal Proceedings and Sentencing

Cataldi initially pleaded guilty in July to healthcare fraud. U.S. District Judge Gretchen S. Lund presided over the case and delivered the sentence on February 18, 2026. While Cataldi faced a maximum sentence of 10 years, Judge Lund sentenced her to 97 months. Her medical license is currently expired, according to reports. Assistant United States Attorney Francis Sohn led the prosecution.

“This sentence reflects that doctors who engage in such outrageous behavior will face significant consequences in this District,” stated U.S. Attorney Adam Mildred in a press release. The case was investigated by the Department of Health and Human Services Office of Inspector General (HHS-OIG).
