El Salvador is significantly expanding access to healthcare through the second phase of its innovative DoctorSV program, a digitally-driven initiative poised to impact an estimated 4.5 million residents. Backed by a $75 million loan from the Corporación Andina de Fomento (CAF), the expansion will focus on decentralizing access to medication and bolstering the platform’s existing telemedicine capabilities. This move underscores El Salvador’s commitment to leveraging technology to address longstanding inequities in healthcare access.
Launched in November 2025, DoctorSV provides free medical attention nationwide through advanced technology, including 24/7 video consultations for non-emergency conditions. The platform already facilitates electronic prescriptions, lab orders, and medication delivery through partner pharmacies, alleviating pressure on traditional hospital settings. The program, a collaboration between the Salvadoran government, Google, and CAF, aims to create a more equitable and efficient healthcare system for all citizens.
Expanding Access Through Medication Kiosks
A key component of the second phase is the planned installation of 400 medication dispensing kiosks in high-population areas. This initiative directly addresses challenges in accessing essential medicines, particularly in remote or underserved communities. According to officials, these strategically placed kiosks will ensure wider availability of medications for Salvadorans across the country. The loan agreement, approved by the Legislative Assembly’s Finance Committee on Monday, February 16, 2026, has a term of 18 years with a 4.5-year grace period and interest rates linked to the SOFR index, as reported by Infobae.
The expansion isn’t limited to infrastructure. The plan also includes training for healthcare personnel to enhance their digital skills and strengthening both the digital and physical infrastructure supporting the telemedicine platform. This holistic approach aims to ensure the long-term sustainability and effectiveness of DoctorSV.
Building on Initial Success and Investment
This latest investment builds upon an initial $77 million credit approved in December 2023, which funded the creation and implementation of the national telemedicine ecosystem. Those initial funds were allocated to cloud storage services, network connectivity improvements, development of clinical and logistical protocols, acquisition of computer equipment, and external audits to ensure responsible leverage of funds. The Ministry of Finance has emphasized that the new funds will centralize health services, enhancing security, interoperability, and equitable access nationwide.
A significant focus of the second phase will be the integration of Artificial Intelligence (AI) and data analytics to optimize information flow and medical processes in real-time. DoctorSV is designed to support healthcare professionals and provide citizens with a convenient channel for medical attention, improving both access and continuity of care. As Pablo Bartol, Social and Human Development Manager at CAF, stated, the initiative fosters “greater equity” and contributes to a stronger sense of national identity, according to El Salvador in English.
Currently, DoctorSV handles approximately 30,000 consultations daily. With the planned expansion, the program anticipates reaching its full potential, benefiting the entire eligible Salvadoran population.
The platform’s success is also reflected in its ability to address inequalities in healthcare access. Bartol highlighted that historically, access to quality healthcare in the region has been stratified by socioeconomic status. DoctorSV aims to eliminate this disparity, making healthcare a true public good accessible to all.
