Dengue Alert: Malaysian Doctors Urged to Watch for Mimicking Symptoms

Kuala Lumpur, June 16, 2025 – Medical professionals in Malaysia are being cautioned to maintain a high level of vigilance for dengue fever, as the mosquito-borne illness increasingly presents with misleading or no symptoms at all. This urgent call comes amid concerns that the disease’s evolving behavior is complicating early diagnosis and treatment.

Rising Concerns Over Dengue Mimicry

Experts emphasize that dengue is now endemic throughout Malaysia,with cases occurring year-round across all states,rather than being confined to seasonal outbreaks. Compounding the problem is dengue’s ability to mimic other common viral infections, such as influenza. Up to 75% of dengue infections might potentially be asymptomatic,creating critically important challenges for early detection and containment.

During a recent panel discussion,Dr.Zulkifli Ismail, Chairman of Dengue Prevention Advocacy Malaysia (DPAM), highlighted a case where a patient initially presented with symptoms suggestive of dengue, including high fever and body aches. However, dengue tests came back negative, and the patient was later diagnosed with influenza.Such cases underscore the diagnostic challenges posed by dengue mimicry.

Dr. Koh Kar Chai, Past President of the Malaysian Medical Association (MMA), stressed that dengue should not be readily misdiagnosed, as it is a intricate disease capable of mimicking other conditions. A high degree of suspicion on the part of medical practitioners is crucial. He also emphasized the critically important role patients play, urging them not to dismiss a fever as “normal”.

The Critical Role of Early Detection

Dr. Zulkifli urged clinicians to maintain open differential diagnoses and highlighted the usefulness of the NS1 test. This test is simple, readily available, and provides results within minutes for a small fee. Early use of the NS1 antigen test is encouraged, as it allows for timely intervention based on subsequent blood tests.

In 2019, the World Health Organization (WHO) listed dengue as one of the top 10 threats to global health.

Dr. Zamberi Sekawi,a clinical Microbiologist at Universiti Putra Malaysia,described dengue as “hyper-endemic,” with cases occurring daily across the country rather than just during perceived outbreaks. He aims to correct the misconception that outbreaks are the sole indicator of the disease’s presence.

Vaccination: A key Strategy

Vaccination is a vital tool in the fight against severe dengue, according to Dr. Zamberi, who chairs the Dengue Vaccine Position Paper Committee. He highlighted the TAK-003 vaccine, which is available in private clinics and hospitals in Malaysia and has demonstrated high effectiveness in preventing symptomatic dengue and reducing hospitalization.

The TAK-003 vaccine has a strong safety profile, backed by 4.5 years of clinical study. Common side effects include mild pain or redness at the injection site. Serious adverse reactions, such as anaphylaxis, are extremely rare. Patients are advised to remain at the clinic for at least 15 minutes after vaccination for observation.
