Honolulu – Health officials in Hawaii have confirmed a case of measles in a vaccinated adult visitor, prompting a public health investigation and exposure alerts across multiple islands. The Hawaii Department of Health (DOH) is working to identify individuals who may have been exposed during the visitor’s travels on Oahu and the Big Island, emphasizing the importance of vaccination and awareness of potential symptoms.

The confirmed case involves an individual who recently arrived in Hawaii from an area of the continental United States experiencing measles transmission, according to the DOH. Despite being vaccinated against measles, the visitor developed the illness and sought medical care even as on Oahu. They are currently recovering in a private residence. This case underscores that while vaccination is highly effective, it doesn’t guarantee 100% immunity, and breakthrough infections can occur.

Exposure Locations Identified

Public health officials have identified several locations where other individuals may have been exposed to the virus. On Oahu, potential exposure sites include Daniel K. Inouye International Airport (HNL) on February 26th (12:30 p.m. – 4 p.m.), March 3rd (9 a.m. – 12:30 p.m.), and March 4th (8:30 p.m. – 11 p.m.). Specifically, the A gates and baggage claim areas were identified as potential exposure zones during those times. The Lāʻie Mormon Temple was also listed as a possible exposure location on February 27th, between 4:30 p.m. And 9 p.m. The Hawaii DOH is urging anyone who was at these locations during the specified times to be vigilant for symptoms.

Exposure locations also extend to the Big Island. Hilo International Airport was identified as a potential exposure site on March 3rd (11:30 a.m. – 2:30 p.m.) and March 4th (6:30 p.m. – 9:30 p.m.), specifically in the gate areas, baggage claim, check-in, security, and gate areas. Hawaii Volcanoes National Park was also listed as a potential exposure location on March 3rd, from noon to 6 p.m., focusing on the Visitors Center and other attractions. Finally, Hilo Siam Thai Restaurant is listed as a potential exposure site on March 3rd, between 5 p.m. And 9 p.m. KITV reports that the infection comes as many have been visiting the volcano due to recent eruptions.

Symptoms and Prevention

Measles is a highly contagious viral illness that can spread through the air when an infected person coughs or sneezes. Symptoms typically begin with a fever, cough, runny nose, and red, watery eyes. A characteristic rash then develops, usually starting on the face and spreading to the rest of the body. According to the Centers for Disease Control and Prevention (CDC), measles can lead to serious complications, including pneumonia, encephalitis (brain swelling), and even death.

The DOH is contacting airlines and airports connected to the visitor’s travel to notify potentially exposed passengers. They are also directly reaching out to individuals known to have had close contact with the confirmed case. Healthcare providers statewide have been issued a medical advisory to be on alert for potential measles cases. Dr. Sarah Kemble, State Epidemiologist, noted that Hawaii Volcanoes National Park, being largely outdoors, may reduce the risk of transmission, but vigilance remains crucial.
