Adenomyosis or endometriosis? Know how to identify the symptoms correctly

Adenomyosis and endometriosis are two conditions that cause changes in the uterus in women. It is common for them to be confused, as the symptoms can be similar. In both, endometrial tissue grows outside of its usual location in the uterus and can cause heavy menstrual bleeding, severe cramps, pelvic pain, and discomfort during sex.

Both conditions can be difficult to diagnose because the symptoms often resemble other gynecological conditions. There are even cases where the two problems occur together. In this case, the treatment can be even more challenging than it already is separately.

Physician Patrick Bellelis, collaborator of the endometriosis sector at the Hospital das Clínicas of the University of São Paulo, explains that the main difference between the conditions is the location of the abnormal growth of the tissue that lines the uterus.

“Adenomyosis is an infiltration of the endometrium, which is the inner layer of the uterus, into the myometrium, the muscular layer of the organ. Endometriosis, on the other hand, occurs when endometrial tissue grows outside the uterus, such as in the ovaries, fallopian tubes or other places in the body, mainly in the pelvic region”, explains Bellelis.

Although the symptoms of the two pathologies are similar, they manifest themselves in different ways. Adenomyosis causes heavier and longer menstrual bleeding, while endometriosis can cause pain during sex, painful bowel movements, and in some cases, infertility.

Adenomyosis usually affects women over 30 years old, who have had children with multiple pregnancies or uterine surgeries. Endometriosis, in turn, can appear in different age groups.

Adenomyosis is diagnosed using imaging tests such as ultrasound or MRI, and a biopsy of uterine tissue may be needed to confirm the diagnosis. “On the other hand, endometriosis can be diagnosed through laparoscopy – a minimally invasive surgical procedure that allows the doctor to visualize the tissue outside the uterus”, highlights Bellelis.

The use of contraceptives can be helpful in controlling the symptoms of both conditions. But more severe treatments like blocking blood flow from a uterine artery for adenomyosis while surgery may be recommended to remove endometrial tissue in endometriosis.
