Harvard researchers tracking 4,884 men with nonmetastatic prostate cancer over a median of 12.8 years found that substituting plant-based fats for animal fats is associated with lower all-cause mortality. Published in JAMA Network Open, the study highlights how post-diagnosis nutrition alters long-term survivorship care beyond initial tumor management.
Dietary fat choices made after a prostate cancer diagnosis are associated with a patient’s long-term survival odds, according to a cohort study of 4,884 men from the Health Professionals Follow-Up Study. Researchers evaluated fat intake every four years through validated food frequency questionnaires, tracking participants through December 2022 to assess how different types of dietary lipids affect all-cause and cause-specific mortality.
Saturated Fat Consumption and Elevated Mortality Risks
During the follow-up period, researchers documented 3,040 deaths from any cause. Men who consumed the highest amounts of saturated and animal fats faced a 24 percent higher risk of dying from any cause compared to those with lower consumption levels.
Data from the cohort showed that participants with the highest saturated fat intake experienced a 26.4 percent risk of death within ten years, contrasted against 23.4 percent for men in the lowest intake quintile. Animal-based fats, which serve as primary sources of saturated fat, showed positive associations with all-cause mortality and deaths stemming from other cancers.

Molecular Pathways Linking Diets to Tumor Growth
Preclinical investigations help explain the biological mechanisms driving these epidemiological findings. The elevated mortality tied to saturated fat consumption stemmed primarily from heart disease and alternative cancers rather than prostate cancer progression itself. Plurality death figures showed that 803 participants died from heart disease.

Nutrient-Density Modeling and Simple Dietary Substitutions
Nutrient-density modeling demonstrated that minor adjustments to daily caloric sources yield measurable survival benefits. Exchanging just 10 percent of daily calories derived from animal or saturated fats with plant-based alternatives like seeds or avocados was linked to a 16 percent lower chance of death within a decade.
Similarly, substituting five percent of daily calories from saturated fat with monounsaturated fats found in olive oil and avocados correlated with an approximate 20 percent reduction in overall mortality risk.

Study authors noted clear limitations, including a participant demographic where 97 percent of the cohort comprised white men, restricting generalization across other populations. Furthermore, individuals consuming higher saturated fat levels were more frequently prone to smoking, elevated body mass index, physical inactivity, or type 2 diabetes.
