What do guidelines recommend?

The guidelines (the American Society for Clinical Oncology [ASCO]European Society for Medical Oncology [ESMO] and the Gynecological Oncology Working Group [AGO]) recommended interventions for the treatment of chemotherapy-induced peripheral polyneuropathy are rare [2, 3, 4].

The best option is to take duloxetine, whose moderate clinical benefit has been demonstrated in a randomized study compared to venlafaxine and placebo [5]. Possible alternatives that may be considered include anticonvulsants and tricyclic antidepressants. The non-pharmacological measures rated positively include physiotherapy and acupuncture. The evidence for scrambler therapy (electronic stimulation of peripheral nerves) is currently not clearly established.

Topics are sometimes rated differently. They fall under the measures that can be considered – sometimes as part of studies. Topical preparations containing menthol, capsaicin or combinations of baclofen/amitriptyline/ketamine or ketamine/amitriptyline were examined. Neither the ESMO, AGO nor ASCO guidelines advocate drug prophylaxis. Cryotherapy can be tried – possibly as part of studies [2, 3, 4].
