[Reporter Wu Liangyi/Report from Taipei]The Centers for Disease Control and Prevention announced today the first local case of cholera in my country this year. It was a man in his 20s. He had eaten sashimi and oysters the day before the onset of the disease, and then developed watery diarrhea several times. and epigastric cramps and seek medical attention, and the test confirmed that the infection was toxigenic Vibrio cholerae.

The Centers for Disease Control and Prevention pointed out that the patient developed symptoms such as diarrhea and abdominal pain on the 28th of last month, and sought medical treatment the next day due to repeated watery diarrhea and upper abdominal cramps. On July 3, a suspected strain was reported due to the culture of feces. On July 5, the test confirmed the infection with toxigenic Vibrio cholerae (serotype O1-Ogawa). At present, the symptoms of the case have improved, and there is no diarrhea.

The Center for Disease Control and Prevention stated that the case had no recent travel history at home and abroad, and his three meals were mostly lunch boxes. He had eaten sashimi and oysters the day before the onset of the disease. One of the co-eaters and three co-residents had no suspicious symptoms. Health units have launched various epidemic prevention and control investigations to clarify possible sources of infection.

According to statistics from the CDC, the cumulative number of local cases from 2019 to 2022 is 0, 1, 0, and 2 cases, and the age distribution is in their 20s to 80s. All of them have been detected with Vibrio cholerae serotype O1-Ogawa; The cases were imported from the Philippines in 2017.

The recent international cholera epidemic has been severe. So far this year, a total of 24 countries have reported cholera cases. Among them, the largest outbreaks in recent years have occurred in eastern and southern Africa. Large-scale cholera outbreaks have occurred in West Asia, Syria, Central America, Haiti, and the Dominican Republic. In addition, Southeast Asia and the Philippines have also reported More than a thousand cases. Due to shortages of medical resources and oral vaccines, the WHO said the risk of global cholera transmission is very high.

Cholera is an acute bacterial enteric infection with an incubation period of usually 2 to 3 days and is transmitted through consumption of contaminated food or water. Because Vibrio cholerae has poor resistance to gastric acid, it usually takes a large amount of bacteria to cause disease. However, in people with insufficient gastric acid, gastrectomy or poor immune function, a small amount of bacteria may cause disease. Symptoms of infection include massive rice soup-like watery diarrhea, vomiting, rapid dehydration, acidosis, and circulatory failure.

The Department of Disease Control and Prevention reminds that because Vibrio cholerae can persist in sewage for a long time, once it is eaten raw seafood (especially crustaceans or shellfish) caught in polluted waters, it may be infected. Therefore, avoid eating raw fish, raw oysters or undercooked shellfish etc. In addition, food should be fully cooked, and cross-contamination of raw and cooked food should be avoided during cooking and preparation; boiled water should be drunk; food that is not to be eaten immediately should be covered and stored in the refrigerator; maintain good personal hygiene habits, before meals, Wash your hands properly with soap and water after using the toilet, before handling food, or after changing diapers to reduce the risk of infection.
