Federal law enforcement authorities have unsealed a sweeping federal indictment targeting four members of a Bronx-based criminal organization known as the “War Room” for their alleged involvement in a massive $12 million Medicaid fraud scheme and associated violent crimes. The operation, which investigators say exploited vulnerable addiction patients and systematically drained taxpayer-funded healthcare resources, relied on sophisticated data falsification and intimidation tactics.
According to announcements made by the U.S. Attorney’s Office for the Southern District of New York and partner agencies, the nine-count indictment outlines a multi-layered criminal conspiracy. The defendants—Louis Trejo, also known as “Machete,” Kenneth Garner, also known as “KG,” Harold Stevenson, also known as “Bazz,” and Erihk Belis, also known as “Eddie”—face serious charges including racketeering, violence in aid of racketeering, firearms, fraud, narcotics, and money laundering offenses.
As alleged by federal prosecutors, the defendants operated a fraudulent enterprise centered around Medicaid-funded transportation services. By fabricating ride data and utilizing GPS spoofing techniques, the network logged fake rides for Medicaid patients to and from methadone clinics in the Bronx. Investigators noted that the group sustained the operation by paying recurring kickbacks to Medicaid patients in cash and drugs, ultimately generating over $12 million in fraudulent Medicaid claims.
Unraveling the “War Room” Racketeering Operation
The criminal enterprise went far beyond simple healthcare billing fraud, according to court documents. Law enforcement officials emphasized that when the organization’s lucrative profits faced threats from rival factions, members resorted to extreme acts of violence to protect their turf. A primary focal point of the violent charges is a January 12, 2024, armed home invasion robbery targeting the leader of a rival fraud ring in Teaneck, New Jersey. During that incident, victims were allegedly restrained, assaulted, cut, and held at gunpoint.
“As alleged, members of the War Room perpetrated a massive fraud scheme that preyed upon the addiction of vulnerable Medicaid patients, defrauded a vital federal healthcare program, and caused over $12 million in losses to American taxpayers,” said U.S. Attorney Jamie McDonald, highlighting the gravity of the multi-agency investigation.
The coordinated takedown involved multiple federal and state entities, including the Justice Department’s National Fraud Enforcement Division, Homeland Security Investigations (HSI), the U.S. Department of Health and Human Services Office of Inspector General (HHS-OIG), the U.S. Postal Inspection Service (USPIS), and the Office of the New York State Comptroller.
Arrests, Court Proceedings, and Ongoing Status
Law enforcement agents executed coordinated early-morning arrests, taking Louis Trejo, Kenneth Garner, and Erihk Belis into custody. The three defendants were scheduled for arraignment before U.S. Magistrate Judge Robert W. Lehrburger in Manhattan federal court. Meanwhile, federal authorities confirmed that Harold Stevenson remains at large, and the overarching criminal case has been assigned to U.S. District Judge John G. Koeltl.
Officials stressed that the operation underscores a dangerous intersection between systemic benefits fraud and violent street networks. “Far from a victimless fraud, this alleged racketeering operation exploited vulnerable patients seeking treatment, stole from a taxpayer-funded healthcare program, and injected narcotics and violence into the criminal enterprise,” stated HSI Acting Special Agent in Charge Pete Gizas.

As judicial proceedings unfold in Manhattan federal court, federal prosecutors and investigative agencies are expected to present further evidence regarding the financial paper trail, the mechanics of the GPS spoofing network, and the broader network of kickbacks tied to the Bronx clinics. Additional updates regarding the status of the remaining fugitive and upcoming court dates will be released as the case progresses.
