note :The right is reserved for children who have never received HPV vaccine before or have received incomplete 2 doses of the vaccine only.
Registration : Must register 3 days in advance of the vaccination service date. Please arrive 30 minutes before the appointment time according to the date and time registered. If you don’t come on time, it’s considered a waiver. Please bring your ID card. and a child vaccination booklet If previously vaccinated against HPV type 2 or type 4 Which can be registered when there is a distance from the first dose of more than 6 months, in case 1 receiving the first dose of HPV 4 types before the age of 15 years will receive 1 more dose of HPV 4 types (if the first dose after the age of 15 years, will receive 2 more doses) In case 2, received 1 dose of the HPV 2-type vaccine, will receive 2 doses of the 4-type vaccine (if over 15 years old, will receive 3 doses)
Persons under 20 years old who register for HPV vaccination must be accompanied by a parent aged 20 years or older.
