Hospitalization at home (HAD) in an accommodation establishment for dependent elderly people (Ehpad) is a system which makes it possible to provide certain hospital care within the establishment with the support of a hospital team. The impact of the intervention of HAD teams in nursing homes on the use and hospital expenses of residents in the short and medium term is examined here by comparing, between 2015 and 2017, the differences in the evolution of residents’ care consumption. from nursing homes which are using HAD for the first time to those establishments which are not using it.

The HAD teams in nursing homes mainly provide palliative care and complex dressings (care of complex wounds: bedsores, complicated ostomies, ulcers, etc.). The intervention of these teams makes it possible to reduce traditional hospitalizations, the use of emergencies not followed by hospitalization, and to increase the use of palliative care at the end of life, while reducing total hospital expenses.. Overall, the HAD contributes to improving the quality of care for residents in nursing homes. However, the intervention of hospital teams to meet essential and routine care needs raises questions of equity of access to care and efficiency, because the HAD offer is not accessible to all establishments, or even to all residents of the same establishment. These results call for questioning the appropriate level of funding for nursing homes to ensure the necessary internal skills while continuing coordination with HAD teams in order to meet all of the health, medico-social and social care needs of residents in Ehpad.
