Researchers in Denmark utilized the country’s nationwide electronic health databases and free healthcare system to examine medical records for approximately 20,000 patients diagnosed with IBD between 2008 and 2018. By looking backward at standard blood tests—evaluating vitamin and mineral levels, blood cell counts, and inflammation markers—the team mapped out how physiology shifts over time.

The analysis showed that biological changes were present in the majority of blood panels up to eight years before a Crohn’s diagnosis, and in more than half of the tests up to three years prior to an ulcerative colitis diagnosis according to the findings. Specifically, white blood cells and platelets ran higher eight years prior to a Crohn’s diagnosis. Seven years out, levels of C-reactive protein (CRP), a classic indicator of inflammation, were elevated, while hemoglobin levels dropped. By five years prior to diagnosis, iron and albumin levels declined while monocyte counts increased. Three years prior to an ulcerative colitis diagnosis, CRP, white blood cells, and platelets all registered higher as detailed in the data.

Crucially, these shifts were so subtle that they fell entirely within standard reference ranges at the time, meaning a treating physician reviewing an isolated lab result would have had no cause for concern based on the study observations.
