Evaluating 40 Clinical Trials Across the Breast Cancer Continuum

A systematic review and meta-analysis evaluated the effects of exercise interventions on fatigue, body composition, physical fitness, and circulating biomarkers in women with breast cancer. Authored by J. Lee and Y. Hwang and published in the Journal of Cancer Survivorship, the research synthesizes evidence from 40 randomized controlled trials comprising a total of 4,271 patients. Among these participants, 2,425 individuals were undergoing active treatment, while 1,947 had completed primary therapies.

The meta-analysis examines how stage-specific physical activity influences patient outcomes. The data indicates that exercise yields distinct physiological benefits depending on whether a patient is actively receiving treatment or navigating survivorship.

Fatigue Management and Fitness Gains During Active Treatment

During active cancer treatment, physical activity directly targets debilitating side effects rather than focusing on weight loss. The pooled data records a statistically significant reduction in cancer-related fatigue among participants undergoing active therapy, alongside notable improvements in physical fitness measured through the 12-minute walk test. However, researchers observed no significant changes in body weight or body mass index during this phase.

Effective programs during active treatment typically last about 12 weeks, incorporating low-to-moderate intensity combined aerobic and resistance exercise.

Post-Treatment Body Composition and Metabolic Biomarkers

Following primary treatment completion, the clinical focus of physical activity shifts toward long-term survivorship, metabolic rehabilitation, and cardiovascular risk reduction. Analyzing data from post-treatment survivors, the meta-analysis reveals substantial improvements in body composition. Lean body mass increased significantly, while fat mass, body fat percentage, and waist circumference decreased markedly.

Post-treatment exercise regimens also favorably modulate circulating biological biomarkers. The review records significant reductions in interleukin-6, low-density lipoprotein cholesterol, fasting blood glucose, and both systolic and diastolic blood pressure, alongside a marked increase in protective high-density lipoprotein cholesterol. Despite these robust metabolic gains, pooled data showed no statistically significant improvements in VO2max cardiorespiratory fitness, handgrip strength, or fasting insulin levels in this cohort.

Despite mounting evidence supporting physical activity, translating research into clinical reality remains a systemic challenge. Kathryn Schmitz, deputy director of UPMC Hillman Cancer Center and professor of medicine at the University of Pittsburgh, has driven multiple sets of exercise guidelines for cancer patients through the American College of Sports Medicine. Reflecting on early skepticism from surgeons when she began her research career around 2000, when rest was considered best, Schmitz highlighted the stark absence of commercial incentives and clinical infrastructure.

Current recommendations suggest combining aerobic exercise—such as walking, cycling, or treadmill workouts at 50% to 85% of maximum heart rate—with resistance training targeting major muscle groups at approximately 60% of one-repetition maximum for the upper body and 80% for the lower body.
