Studies have shown that diabetes medications are effective in alleviating the symptoms of non-alcoholic steatohepatitis.

Severance Hospital’s endocrinology department professors Cha Bong-soo, Lee Yong-ho, and Lee Min-young and hepatobiliary and pancreas surgery professor Han Dae-hoon’s research team announced on the 28th that ‘SGLT-2 inhibitor’, which is prescribed to diabetic patients to drop blood sugar, reduces glucose accumulation in hepatocytes and alleviates hepatitis.

The results of this study were published in the latest issue of Metabolism: Clinical and Experimental (IF 9.8), an international journal in the field of endocrine metabolism.

If you suffer from non-alcoholic steatohepatitis, in which fat accumulates in the liver and becomes damaged and inflamed, the probability of developing cirrhosis within 10 years is up to 29%. When cirrhosis is present, the incidence of liver cancer rises to 27%.

Non-alcoholic fatty liver disease is a common liver disease with a prevalence of up to 20% of the world population, but there is no treatment approved by the US Food and Drug Administration (FDA) yet.

The research team analyzed the liver tissues of 29 patients with non-alcoholic fatty liver disease and 15 patients without non-alcoholic fatty liver disease among patients who underwent liver resection or cholecystectomy for liver cancer or cholecystitis at Severance Hospital for four years from 2015.

As a result, it was confirmed that SGLT-2, a protein that transports sugar into cells, and intracellular sugar-binding proteins increased in the liver with non-alcoholic steatohepatitis.

Accordingly, the research team hypothesized that the SGLT-2 inhibitor, a diabetes drug that lowers blood sugar, would alleviate non-alcoholic steatohepatitis by reducing excessive sugar intake into hepatocytes.

Subsequently, mice induced with non-alcoholic steatohepatitis were observed, and the mice showed the same protein changes as those in patients with actual non-alcoholic steatohepatitis.

In addition, it was confirmed that the autophagy of hepatocytes, which clean diseased tissues and wastes on their own, decreased in mice and the range of liver inflammation widened.

The research team interpreted this as a result of the additional sugar binding to the protein responsible for autophagy, resulting in a loss of original function.

Afterwards, when the SGLT-2 inhibitor was administered, the amount of SGLT-2 protein expression and sugar-linked protein decreased in the liver of mice, as well as the autophagy of hepatocytes was restored and the inflammatory response was alleviated. The research team explained that these changes were the process by which the symptoms of non-alcoholic steatohepatitis were alleviated.

Professor Bong-Soo Cha said, “This study is meaningful as it is the first study to suggest medical evidence and mechanism that SGLT-2 inhibitors for diabetes treatment alleviate the symptoms of non-alcoholic steatohepatitis.” It is expected to improve the prognosis of non-alcoholic steatohepatitis, which is on the rise, and prevent deterioration to liver cirrhosis and liver cancer.”

▲ Professor Gunho Ban of the Department of Psychiatry at Kyung Hee University Hospital translated and published ‘Female ADHD-From Invisible Girl to Burnout Woman’.

Professor Ban Geon-ho of the Department of Psychiatry at Kyung Hee University Hospital, one of the leading experts in ADHD research in Korea, translated ADHD: Från duktig flicka till utbränd kvinna, a book written by Dr. Published ‘As a Burnout Woman’.

According to data from the Health Insurance Review and Assessment Service, the number of female ADHD patients in Korea has increased rapidly over the past five years, exceeding 30,000 per year. In particular, the number of female patients in their 20s and 30s increased noticeably.

The author explains the differences in ADHD according to gender and emphasizes the importance of research on female ADHD and differentiated approaches according to gender.

Looking at the representative differences, women may show symptoms ups and downs according to the menstrual cycle, and inattention symptoms may be more prominent than hyperactivity or impulsivity due to the characteristics of women.

Professor Geonho Ban of Kyung Hee University Hospital said, “ADHD can last from childhood to adulthood, and if proper diagnosis and treatment are not provided, it can lead to social maladjustment such as low self-esteem and depression.” , women), so that individual characteristics differences can be easily understood in the clinical field and treatment that reflects them can be preceded, further emphasizing the need for the development of ADHD treatment tailored to women.”

◇Gangdong Kyunghee University Hospital Professor Park Chang-beom published ‘Doctor Standing in Court’

▲ Professor Park Jeong-beom.

Kangdong Kyung Hee University Hospital Cardiovascular Medicine Professor Park Chang-beom published a medical law common sense book ‘Doctor Stands in Court’.

If you look at these two words, doctor and law, it is easy to think of lawsuits caused by medical malpractice. But doctors face legal challenges in more situations than you might think.

‘Doctor Standing in Court’ summarizes common sense of the law that doctors should know as medical personnel and members of society.

The part you need to know as a doctor contains most of the legal common sense necessary for doctors, from legal common sense you need to know while treating, to essential content such as medical advertisements and claims for medical care benefits, and sensitive content such as rebates and false medical treatment.

Not only medical workers, but also legal common sense that you need to know as a member of society, such as work with a superior during work, retirement, and profit-seeking hospitals, and a wide range of ethics-related content such as abortion and Good Samaritan issues.

Professor Park Chang-beom said, “Many doctors think that law is too difficult and do not look into it.” I wanted to show that it works,” he said.

He continued, “I hope that it will be of some help to doctors and medical personnel who live fiercely by treating patients every day.”

◇ In Jae-geun “non-face-to-face prescription with a hole, concerns about drugs and abuse”

In the case of non-face-to-face treatment, which was temporarily allowed in the Corona 19 situation, it was pointed out that there were not a few cases of illegal prescription despite the restriction on prescription of certain medicines such as narcotics and medicines of concern for abuse from November 2, 2021.

In addition to the Health and Welfare Committee of the National Assembly, Rep. In Jae-geun of the Democratic Party analyzed the data recently submitted by the Ministry of Health and Welfare, and revealed that certain drugs, such as narcotics and medicines concerned with misuse and abuse, that were banned from non-face-to-face prescription during the COVID-19 period were widely illegally prescribed.

From February 24, 2020, the Ministry of Health and Welfare temporarily allowed non-face-to-face treatment in consideration of the COVID-19 situation.

Since then, it has been pointed out that the temporary non-face-to-face treatment is not operated according to its original purpose, such as that it is possible to easily prescribe sexual function enhancers and diet pills in some non-face-to-face treatment platform applications.

Accordingly, from November 2, 2021, the Ministry of Health and Welfare restricted the prescription of ‘prescription-restricted medicines’ through non-face-to-face treatment.

However, as a result of Congressman In’s analysis of the data submitted by the Ministry of Health and Welfare, 46,650 examinees for about 14 months from November 2, 2021 to December 31, 2022 were prescribed ‘prescription-restricted medicines’ covered by health insurance benefits. appear.

The number of prescriptions reached 58,495, of which about 5% (2,993 cases) were prescribed to people under the age of 19.

By type, diazepam (28.0%) was the most common, followed by alprazolam (16.8%) and zolpidem tartrate (12.6%).

In this way, “limited prescription drugs” were prescribed indiscriminately, but there was only one case (March 2023) in which the Ministry of Health and Welfare actually detected a case and imposed a fine.

The Ministry of Health and Welfare’s position is that if it is confirmed that a medical institution has prescribed ‘prescription-restricted medicine’, the health insurance review and assessment service has completely reduced health insurance benefits through computerization and reviewer adjustment. It is pointed out that there is no change in the fact that it cannot be recovered.

Rep. In Jae-geun said, “Drugs that can cause serious side effects are prescribed too easily, and the cases confirmed through this data from the Ministry of Health and Welfare are limited to ‘prescription-restricted medicines’ to which health insurance benefits apply.” It is not even possible to check the prescription status of medicines,” he pointed out.

Then, “It was revealed that the Ministry of Health and Welfare was neglecting the management and supervision of non-face-to-face prescriptions.” We need to find ways to strengthen management and supervision,” he stressed.
