The standard diagnostic pathway for individuals suspected of having prostate cancer typically involves an initial magnetic resonance imaging (MRI) scan, followed by a decision regarding tissue acquisition. Traditional biopsies require the insertion of a fine needle into the prostate gland to extract tissue samples. Furthermore, biopsies can sometimes fail to detect some tumors or detect tumors that may not need treatment.

To evaluate whether a subset of patients could bypass this invasive step safely, researchers at UCLH conducted an evaluation of 880 men referred to a specialized diagnostic clinic over a one-year period. All participants underwent MRI scans. A panel of four radiologists reviewed the imaging data alongside patient age and clinical results. Among the cohort, 251 patients were stratified as having the highest probability of malignancy, and 217 subsequently underwent biopsies to verify the imaging findings.

The results, published in the European Journal of Radiology, revealed concordance between imaging predictions and histological outcomes. Out of 69 patients whose MRI scans were categorized by the panel as indicating a 99% probability of malignancy, surgical biopsies confirmed the presence of cancer in all of them. In a separate tier of 45 patients assessed with a 90% probability score, 41 underwent biopsies, yielding significant tumors in 40 cases, representing a 98% rate.
