these health issues on which the government is expected

Rarely have the spotlight been so focused on health spending, on the eve of the presentation to the Council of Ministers, scheduled for Wednesday September 27, of the Social Security financing bill (PLFSS). Among the savings envisaged by the executive, these are the first to be targeted. Even on the ground, players in the sector – hospitals and private doctors in the lead – are closing ranks to make their urgent needs heard.

To achieve savings of between 10 and 15 billion euros in 2024, Bercy put forward the first ideas in June: reducing sick leave requirements and doubling medical deductibles on boxes. of medicines. Dividing measures, but which are far from exhausting, in themselves, the subjects on which the government is expected.

An Ondam up to the task?

The acronym does not speak to the general public, but the national health insurance expenditure objective (Ondam) – i.e. the envelope voted by Parliament for city, hospital and medico-social care – symbolizes each year the fate reserved for the world of health in financial terms. Aurélien Rousseau, the Minister of Health, has already mentioned an increase of + 3.2%. Barely more than the 3% mentioned in the press by the Minister Delegate for the Budget, Thomas Cazenave. For the record, in 2023, this Ondam rose by + 3.7%.

The evolution for 2024 is far from meeting expectations. « Discussing an evolution of the Ondam in nominal terms, that is to say without taking into account inflation, does not make sensepoints out the general inspector of social affairs Pierre-Louis Bras (also president of the Retirement Orientation Council). Given the very high inflation rates of the current period – 5.8% for example forecast in 2023 – the objective seems particularly constrained. »

At the French Hospital Federation, the public hospital lobby, we would like a much higher Ondam: an increase of 5 billion euros is considered necessary, an increase of 4.7% compared to 2023. Revaluation measures night work in hospitals, announced by the government on August 31, to the tune of 1.1 billion euros, will have to find their place in these budgetary tables.

The overall envelope must also cover the increases promised to private doctors, while on 1is November, the price of a consultation with a general practitioner must increase from 25 euros to 26.50 euros. A ” stage “, promised the general director of Health Insurance, Thomas Fatôme, without committing at this stage to the timetable for resuming conventional negotiations with the sector’s unions. They already have a call for an indefinite strike for October 13.
