TSA: for a better life course for children and adolescents

This framework note published by the Haute Autorité de Santé (HAS) contains information relating to the theme “ Autism spectrum disorder (ASD): interventions and life course of the child and adolescent “. This is an internal request from the HAS to update the good practice recommendation (RBP) ” Autism and other pervasive developmental disorders: coordinated educational and therapeutic interventions in children and adolescents » published in 2012.

The objective of the future recommendation of good practice (RBP) is to improve the quality of the interventions offered to children with ASD, both by health and medico-social structures, in order to allow them to benefit from a coherent and quality life course, as well as to promote their access to learning and inclusive living environments. If its vocation is to take into account the recent evaluations of the intervention methods recommended in 2012 and to analyze the evidence of new interventions or various forms of support, it is now a question of having clinical benchmarks making it possible to adjust interventions to the developmental profile of each child, to their learning pace, and possibly by mixing different tools and approaches.

In particular, for these children, adolescents and young adults with ASD, through standardized and shared functioning assessments, this involves:
‒ to improve their operation;
‒ reduce their associated health problems;
‒ help them develop their ability to communicate, choose and decide;
‒ support their autonomy in daily life;
‒ to promote their school and educational learning;
‒ to include them in an appropriate professional project;
‒ to have a satisfactory quality of life;
‒ develop their social participation and prepare them to exercise full citizenship

The affected population includes:
– any child and adolescent up to the age of 20, with ASD, as soon as the diagnosis of ASD is made; – the child with suspected ASD confirmed during the dedicated identification consultation in primary care, pending the specialist consultation with 2nd line professionals.

What are the issues ?

For professionals :

‒ improve and harmonize support methods and care practices, taking into account the specificities of children with ASD;
‒ influence the developmental trajectory of children through early intervention;
‒ promote the generalization of learning by involving parents in the development of their child;
‒ ensure better coordination of the various interventions;
‒ take care to prevent any interruption in the support and care pathway, with attention to the transition from adolescence to adulthood.

For children and teenagers

‒ promote their development and their learning in the areas concerned by ASD by implementing appropriate interventions, integrated into an evolving life course, leaving room for self-determination, in a more inclusive society (sport, culture, hobbies).

For legal representatives, parents, siblings and family carers

– co-construct with them the various projects (care, support, schooling) and develop co-intervention;
‒ Prevent the exhaustion of caregivers and offer appropriate support methods to preserve family balance and the quality of intra-family relationships.

This RBP is intended for professionals who are in direct contact with children, adolescents and young adults with ASD in their various activities and places of life, or who intervene in their follow-up and support. The other recipients of the RBP are minors or young adults with ASD and their entourage.
