Understanding the Impact of Endometriosis on IVF Success Rates: A Comprehensive Study

Why is this important?

About 10% of women of childbearing age may have endometriosis. This rate would reach 30 to 50% in those with fertility disorders. The impact of endometriosis on the birth rate of women undergoing IVF remains debated within the scientific community.

Methodology

This retrospective cohort conducted at La Conception Hospital in Marseille focused on women with moderate to severe endometriosis included in an IVF or intracytoplasmic sperm injection (ICSI) process between January 2015 and December 2020. They were matched 1:2 with women with other causes of infertility (control group).

The primary endpoint was the cumulative live birth rate (cTNV) per cycle per woman. Other secondary endpoints were measured such as number of oocytes retrieved, number of mature oocytes, fertilization rate, total number of embryos and usable embryos, implantation rate, pregnancy rate clinics and miscarriage rate per cycle.

Principle results

This study included 585 women. The data of 195 women with endometriosis were compared with those of 390 women who were free (ie 323 and 646 cycles respectively). For the women in the first group, the mean age was 33.1 years, the mean BMI 23.5 kg/m2 and the mean concentration of anti-Mullerian hormone 2.5 ng/ml.

On average, women had undergone 1.7 cycles of IVF regardless of group. The characteristics of the women were similar between the two groups at inclusion apart from the primary infertility rates which were significantly higher in the group of women with endometriosis (65.5% versus 53,3%).

The cumulative live birth rate per cycle and the live birth rate per woman were comparable between women with endometriosis and control women (19.8% versus 24.3%, p=0.12 and 32.3% versus 37.2%, p=0.24 respectively).

The number of oocytes retrieved was significantly lower in women with endometriosis compared to those in the control group, despite higher doses of gonadotropins (3,189.5 versus 2,750 IU, p<0.001) among women with endometriosis. En revanche, the rates of maturation and fertilization were comparable between the two groups (73% versus 72% et 62% versus 60% respectively for women with endometriosis and control women).

Furthermore, the total numbers of embryos and usable embryos were comparable between the two groups. The number of oocytes retrieved in women with endometriosis was similar in women with a history of surgery and those without.
