Eleven carriers have announced plans to exit the Affordable Care Act (ACA) Marketplaces for the 2027 plan year, affecting coverage across multiple states following the expiration of enhanced premium tax credits.
In Plain English: The Clinical Takeaway
- Coverage Shifts: Insurance carriers are pulling out of several state Marketplaces for 2027, which will reduce the number of health plan choices available to consumers in affected regions.
- Market Risk Pools: When healthier enrollees drop coverage, the remaining patient population can become sicker on average, prompting more insurers to reassess their participation.
Carrier Departures and New State Entrants for 2027
Beginning in plan year 2027, 11 insurance providers have stated they will leave the ACA Marketplaces across some or all of their current operating states, according to data available through October 28, 2026. Concurrently, seven carriers have announced they will enter new state Marketplaces.

These shifting participation patterns follow a drop in ACA enrollment. Sign-ups fell by over a million from the 2025 to 2026 Open Enrollment Periods. This contraction followed the expiration of the enhanced premium tax credits at the end of 2025.
Cigna Exit and Individual Market Strategy
Among the exiting insurers, Cigna has decided to leave the individual market in 2027 to focus on other segments. The company reported first-quarter on-exchange enrollment of over 350,000 individuals.
Cigna’s decision stems from a lack of potential to grow their ACA Marketplace business. Cigna will exit the 11 states in which it currently participates both on- and off-exchange.
Risk Pool Dynamics and Consumer Choice
When enrollment drops within the ACA Marketplaces, it shrinks the addressable market for insurance providers and can alter the risk pool because healthier participants tend to drop coverage more frequently.
Future departures by insurers could be triggered if customer reductions continue to impact the financial viability of offering Marketplace plans. Additionally, reduced insurer participation in specific states will naturally diminish market competition and restrict consumer options.
Contraindications & When to Consult a Doctor
Anyone undergoing active treatment should verify that their current physicians and prescription formularies are covered under any newly selected Marketplace plan.
