A mission to support the generalization of the Care Access Service (SAS)

On April 24, François Braun, Minister of Health and Prevention, announced the launch of a mission responsible for supporting the generalization of the Access to Care Service (SAS) in all regions of France.

For the Minister of Health and Prevention, François Braun, the generalization of Care Access Service launched in June 2022, “ is a fundamental issue to support the reorganization of access to an offer of unscheduled care adapted to the needs of French people“. In a context of tensions on the health system, the importance of the deployment of the Care Access Service (SAS) was reaffirmed in the summer of 2022 as part of the flash mission for emergencies and unscheduled care after a pilot phase conducted in 22 territories. On October 3, 2022, at the end of the launch day of the National Council for Refoundation in HealthFrançois Braun underlined that any future overhaul of the health system will involve “ that at any time of the day, citizens can find an answer for unscheduled care. For this, it seems essential to generalize the service of access to care from the year 2023“. Likewise, during wishes to health actors on January 6, 2023the President of the Republic affirmed “ I hope that we can accelerate the generalization of the healthcare access service during this year« .

A new response to the health needs of the French people

The attending physician remains the pivot of patient care, for regular follow-up as well as for unscheduled care. The territorial communities of health professionals also provide additional local responses, including helping to find a general practitioner for patients who do not have one. The SAS intervenes at the end, that is to say when access to the attending physician is not possible and the patient has not found any other solution on his territory. The SAS allows everyone to obtain, via a simple call to 15, an immediate response
in case of emergency and orientation in the health system when he cannot find a solution to a health problem. When you have not found a solution on your territory, or when you are far from home and you do not know the solutions available nearby, calling 15 must become the new reflex of the French to know the answer most adapted to their health needs. With the SAS, it is the system that organizes itself to guide the patient towards the solution best suited to the reality of his health needs. By providing the public with a service
dedicated, the SAS contributes to limiting the use of emergencies when this is not necessary.

“Naturally, this organization does not come at the expense of the SAMU handling emergency calls, which continue to be handled without delay and as an absolute priority. This point is also the subject of closer monitoring in conjunction with all the parties involved. »

Offer this service to all French people by the end of 2023

After this successful pilot phase, the deployment of this service has started and should cover the entire national territory by the end of 2023. While a growing number of our fellow citizens live in a medical desert, it is a measure
important in the fight against social and territorial inequalities in health. To accelerate this deployment and cover all territories, an SAS generation support mission has been launched. This mission will move
throughout France until the summer of 2023 to draw inspiration from successful experiences and thus enable the deployment of SAS in all territories in conjunction with existing local organizations.
This assignment is made up of:
– a liberal representative: Dr Yannick Frezet (general practitioner in Rive-de-Gier);
– a hospital pair (each part-time): Dr Yann Penverne (emergency doctor at Nantes University Hospital) and Dr Henri Delelis-Fanien (emergency doctor at Poitiers University Hospital);
– a patient representative who will soon be appointed by France Assos Santé;
– a representative of a regional health agency: Ms Céline Etchetto, (director delegated to the organization of the care offer of ARS Nouvelle-Aquitaine).

The tools for generalization are being put in place:
– The decree providing for the generalization of the SAS is currently in consultation. It will make it possible to establish the regulatory framework of this new system and to secure the organisations.
– The national platform is operational. It is the computer tool essential to the generalization of the SAS: it lists all the unscheduled care offers available in a territory and allows the SAS regulator
direct callers.
