Ethanol, the primary active ingredient in alcoholic beverages, metabolizes within the body into acetaldehyde, a toxic chemical compound capable of damaging DNA and impairing cellular repair mechanisms. The burden of alcohol-attributable cancer deaths concentrates heavily among men and adults aged 55 and older. In 2023, individuals aged 55 and older accounted for 19,460 of the total 23,126 deaths—roughly 84 percent of the national toll. This demographic distribution aligns with the natural history of oncology; because cancer possesses a long latency period, cumulative lifetime exposure heavily influences risk profiles in older populations.

Researchers noted diverging trends between general cancer mortality and alcohol-specific figures. While overall cancer mortality rates dropped substantially over the 33-year study window due to improvements in prevention, screening, and treatment, alcohol-linked deaths declined much more slowly or increased in specific cohorts. Among women, overall cancer mortality fell by about 29 percent between 1990 and 2023, whereas alcohol-attributed cancer deaths decreased by only 17 percent. Among men, overall mortality dropped 34 percent, while alcohol-related fatalities decreased by just 14 percent.
Shifting Risk Profiles Among Younger Adults
For adults aged 20 to 54, the leading causes of alcohol-attributable cancer death diverge from older age groups, with colorectal cancer emerging as a critical concern. Among men in this younger cohort, colorectal cancer ranked as the leading alcohol-related mortality cause, followed by esophageal cancer. For women aged 20 to 54, breast cancer claimed the top spot, with colorectal cancer ranking second.
Study authors emphasized that colorectal cancer surpassed liver cancer among these younger demographics, reflecting a broader rise in early-onset gastrointestinal malignancies.
