A Pakistani baby was born with a rare malformation: he was born without an anus and with two functioning penises. The occurrence of two penises is so extraordinary that only 100 similar records have been found in the history of medicine, with the first case dating back to the 17th century.
The report about the baby was published in the International Journal of Surgery Case Reports last March. Diaphilia, as the condition of having two penises is called, is an anatomical anomaly that occurs once in every six million births.
“Diaphilia is often associated with other congenital malformations, such as an open urethra, a displaced scrotum in the body, a duplicated or out-of-body bladder, a closed anus, and even malformations of the vertebrae,” the doctors described.

The baby was born vaginally after 36 weeks of gestation. The penises were not equal to each other, one being 1 centimeter larger than the other. The two had all urinary functions established and, by all indications, were also fit for reproductive functions.
The baby underwent postpartum emergency surgeries to open the rectal canal. The two penises were kept, but the child will undergo periodic examinations. “It is necessary to evaluate which is the least functional penis to start plastic surgeries”, stated the doctors.
As anatomically both penises appear functional, further study will be needed to determine which one will be removed.
