On September 12, 2022, a rocket from the US company went off Blue Origin burst into flames shortly following takeoff. The capsule attached to the nose of the rocket successfully initiated an emergency separation sequence and floated safely to the ground on parachutes. The US Federal Aviation Administration (FAA) investigated the accident and concluded that engine nozzle failure was responsible for the failure.
