An ongoing U.S. cyclosporiasis surge has sickened an estimated 18,000 people across 45 states since May, with at least two deaths reported in Michigan and food safety officials tracing infections to shredded iceberg lettuce.
The parasitic intestinal infection is driven by Cyclospora cayetanensis, a microscopic organism typically contracted through contaminated food or water. While annual cases normally don’t surpass 4,000, this summer’s totals have shattered previous benchmarks. Federal health authorities point to a wide-scale outbreak spanning nine states in the Midwest and South, while investigating multiple smaller, separate clusters nationwide.
National Toll Reaches 18,000 Cases as Midwest and South Bear the Brunt
Nearly 18,000 confirmed or probable cases of cyclosporiasis have emerged since May 1, according to data from the U.S. Centers for Disease Control and Prevention. The federal agency has confirmed 6,707 laboratory-positive infections nationwide, while more than 11,500 additional cases remain under investigation as officials rule out international travel and trace exposures.

The outbreak is hitting the Midwest and South especially hard. A primary nine-state cluster links infections across Indiana, Kentucky, Ohio, Michigan, West Virginia, Illinois, Kansas, Oklahoma, and Pennsylvania.
State health agencies across the country report varying figures because they include probable cases that have not yet cleared federal laboratory verification.
Lettuce Recall Explains Only a Fraction of Summer Infections
Federal investigators identified shredded iceberg lettuce supplied by Taylor Farms de Mexico and served at Taco Bell as the source of one distinct outbreak comprising 1,947 confirmed cases across nine states. Taco Bell removed the implicated produce from its supply chain, and Taylor Farms issued a voluntary recall.
However, that recall leaves a massive statistical gap. The lettuce-linked cluster accounts for only a fraction of the illnesses reported this summer. Experts emphasize that the national numbers reflect a broader systemic distribution rather than a single tainted food item.
Food safety specialists explain that the parasite is carried in human feces and contaminates fresh fruits and vegetables when grown in tainted soil or washed with contaminated water. Person-to-person transmission is very unlikely because the parasite requires one to two weeks outside the human body to mature and become infectious.
False-Positive Lab Results and Regulatory Complications
The investigation faced a procedural hurdle when the U.S. Food and Drug Administration retracted a laboratory finding. A sample of Taylor Farms shredded iceberg lettuce initially tested positive for the parasite, but federal regulators later determined the result was a false positive caused by the difficulty of detecting the parasite.

Despite the retraction, federal officials maintained that the ongoing investigation rests on solid epidemiological foundations. Taylor Farms stated that regulatory traceback points to a single independent farm in central Mexico representing less than one percent of the U.S. iceberg supply, from which all product was removed as a precaution.
The two fatalities reported thus far occurred in Michigan. State health officials confirmed that both individuals suffered from significant underlying health conditions exacerbated by cyclosporiasis and dehydration.
Public Health Warnings and What Remains Unresolved
Federal guidance to clinicians highlights that the true number of illnesses is likely higher than what has been reported,
largely because many infected individuals recover without seeking medical care or undergoing specific diagnostic testing. A standard six-week reporting lag separates symptom onset from inclusion in federal databases.
With fresh produce season continuing through August, investigators must still determine whether unlinked clusters share common agricultural origins or represent entirely separate contamination events.
