CRE Placed at Forefront of Clinical Concern in Study

In Plain English: The Clinical Takeaway

  • Multidrug-Resistant Organisms (MDROs): Bacteria that have developed resistance to three or more distinct classes of antibiotics.
  • Carbapenem-Resistant Enterobacterales (CRE): A family of bacteria resistant to carbapenems.
  • Colonization vs. Infection: Colonization means the bacteria exist in the patient’s body without clinical symptoms or tissue invasion, whereas infection involves the bacteria invading the body, multiplying, and causing clinical symptoms and tissue invasion.

The study assessed point prevalence, colonization rates, infection statuses, infection severity, and treatment feasibility across four distinct survey periods in April 2024, July 2024, October 2024, and January 2025. The resulting evaluation placed CRE at the forefront of clinical concern.

Vancomycin-resistant enterococci (VRE) and multidrug-resistant Acinetobacter baumannii (MRAB) followed CRE in priority ranking. Methicillin-resistant Staphylococcus aureus (MRSA) and multidrug-resistant Pseudomonas aeruginosa (MRPA) ranked lower on the severity and burden index. However, researchers noted that because the study could not independently isolate the attributable mortality of each pathogen, VRE’s overall clinical risk profile may have been artificially elevated in initial scoring models.

Colonization Rates Versus Active Infection Metrics

Data tracking the distinction between asymptomatic carriage and active pathology revealed clear epidemiological splits among pathogens. For colonization—where bacteria reside in or on the host without tissue destruction—CRE demonstrated the highest point prevalence at 1.79 percent, closely followed by VRE at 1.73 percent and MRSA at 1.59 percent.

Conversely, when measuring active infection prevalence involving tissue invasion and systemic symptoms, MRSA led at 0.53 percent. CRE active infections sat at 0.30 percent, followed by VRE at 0.23 percent and MRAB at 0.22 percent.

Pathogen Colonization Prevalence Active Infection Prevalence 30-Day Infection Mortality Median Cost Per Infection
CRE 1.79% 0.30% 16.9% 4천600만원
VRE 1.73% 0.23% 18.6% 5천900만원
MRSA 1.59% 0.53% 12.4% 2천900만원
MRAB – 0.22% – –

When examining infection severity among active cases, VRE registered a 30-day mortality rate of 18.6 percent, while CRE cases logged 16.9 percent and MRSA recorded 12.4 percent. Research teams emphasized that these figures reflect underlying patient comorbidities and severe immunosuppression rather than direct, single-cause mortality driven by the pathogens alone.

Regarding treatment and clearance, MRAB demonstrated the highest clearance rate, with 60.3 percent of cases successfully eliminating the bacteria and clearing management within six months. MRSA and VRE achieved clearance release rates of 58.6 percent and 53.8 percent, respectively.

Economic Costs and Institutional Variations

Financial evaluations highlighted substantial economic burdens associated with managing these hospital-acquired infections. Median medical expenses per infection ranged from a low of 2천900만원 for MRSA up to 5천900만원 for VRE. CRE infections carried a median medical expenditure of 4천600만원 per episode.

The institutional audit also revealed sharp disparities in internal hospital resources. While all 21 surveyed medical facilities maintained infectious disease specialists on staff, only one institution—representing five percent of the surveyed group—employed a dedicated infection control physician.

Future Public Health Policy and Regional Networks

Building on these findings, the KDCA intends to expand targeted CRE reduction protocols and strengthen regional medical institution networks dedicated to healthcare-associated infection control. KDCA Commissioner Lim Seung-kwan stated that multidrug-resistant bacteria represent a critical global threat to patient safety, noting that scientific data confirms the immense disease burden of CRE and underscores the necessity of prevention-focused management to secure safe clinical environments.
