In case of authorizing another person to receive one-time social insurance on behalf of the person, the dossier includes: The completed social insurance book, ID card or CCCD of the authorized person, an application form (made according to form 14-HSB).
In addition, a power of attorney form No. 13-HSB (issued together with Decision No. 166/QD-BHXH of Vietnam Social Security) with attested signature of the authorized person of the local government or contract of authorization is required. the right to be established in the notary’s office.
