A 39-year-old woman reported that after piercing her ears, she began to feel itching and pain, and a dark red mass quickly grew to cover her piercing. Because the pain had affected her life, she went to a dermatologist. After examination, it was found that the dark red mass was the outward expansion and hyperplasia of the scar, which in turn caused “keloid”.

Keloids that do not disappear over time and affect appearance and are accompanied by discomfort

Luo Zikun, director of the Dermatology Department of Annan Hospital, said that keloids are abnormal scar tissue that usually form after skin injury or trauma. In addition, unlike normal scars that flatten and disappear over time, keloids thicken and become raised, and may even continue to expand to areas other than the original wound. In addition to affecting the appearance, it may also cause discomfort, itching and pain.

The cause of common keloids is easy to form if you look at heredity once

Keloids can form anywhere on the body, but are most common on the chest, shoulders, earlobes, and back. Director Luo Zikun explained that keloids may occur in any skin wound, including cuts, burns, surgical incisions, vaccineInoculations, and even acne, result from the body’s natural healing process overreacting, meaning excess collagen is produced, which promotes the growth and protrusion of scar tissue, thus forming keloids. Luo Zikun added that some patients are more likely to form keloids than ordinary people, which may be related to “heredity”.

Keloid treatment is quite tricky and common in 5 ways

For this reason, dermatologists usually diagnose keloids through a physical examination and, if necessary, a skin biopsy. Luo Zikun mentioned that the treatment of keloids is quite challenging, because the general scar treatment methods (such as topical ointment and silicon film) are often not effective, but there is still a risk of recurrence when choosing excision surgery. He suggested that surgical resection should be considered unless other treatment methods have not been effective, and other treatment methods need to be combined to effectively reduce the risk.

For other treatments, five methods can be considered, such as steroid injections, cryotherapy, laser therapy, photodynamic therapy, and radiation therapy:

1. Steroid injections:Also known as de-scar injection, it is the first choice for the treatment of keloids. It can be injected directly into the lesion to reduce inflammation and shrink scar tissue, but multiple injections may be required for several weeks or months

2. Cryotherapy:Freezing keloids with liquid nitrogen, which can help reduce the size of the keloids, but may also cause loss or precipitation of skin pigment

3. Laser therapy:Fraxel laser is used to break down excess collagen to reduce the size of the scar, which may require multiple sessions over several months

4. Photodynamic therapy:Apply light-sensitive drugs to the lesion and irradiate light with special wavelengths, which also requires multiple courses of treatment

5. Radiotherapy:Radiation of keloids, usually reserved for patients with large keloids or poor response to other treatments, or excisional surgery combined with radiation therapy

Keloid is difficult to prevent and avoid unnecessary skin trauma is the best policy

Director Luo Zikun explained that although keloids are difficult to prevent, measures can be taken to reduce the risk, such as avoiding unnecessary skin trauma, keeping the wound clean and properly covered, avoiding piercing or tattooing in areas prone to keloid formation, and seeking timely medical treatment for acne or other skin diseases.
