uniQure faces 36-month confirmatory trial for Huntington’s gene therapy

uniQure’s investigational gene therapy ifezuntirgene inilparvovec faces a May 1, 2027 Prescription Drug User Fee Act action date under the U.S. Food and Drug Administration's Accelerated Approval pathway, triggering a demanding 36-month confirmatory trial protocol in Huntington's disease that clinical operations teams must manage alongside simultaneous commercial rollouts.

Trial Burdens Strain Specialized Neurological Centers

  • The Trial Burden: A 36-month endpoint means every enrolled patient must complete structured assessments across three years, a significant challenge in progressive neurological disorders.
  • The Crossover Conundrum: Sites running randomized trials face immediate protocol amendments and crossover requests when experimental therapies receive commercial approval.
  • The Resource Gap: Specialized neurosurgical centers must balance post-procedural monitoring against surging commercial access demands using standard site budgets.

Confirmatory Commitments Versus Planning Assumptions

The site initiation visit is done. The investigational product is delivered, the neurosurgical team is trained, and the first patient is dosed.

Sites working with rare neurological indications call that timeline a planning assumption. The U.S. Food and Drug Administration calls it a confirmatory commitment. The gap between those two descriptions is where trials collapse.

The agency accepted the Biologics License Application after an earlier reversal in which officials initially signaled the Phase I/II data was insufficient, then changed course in November 2025.

What emerged from that regulatory negotiation is a confirmatory trial design that checks every box the agency wants to see. At the same time, it creates a set of operational problems that the protocol’s risk register almost certainly underweights.

Patient Retention Complicates Confirmatory Trial Enrollment

That patient count was workable for a small-site, specialist-center model.

The confirmatory trial target of 200 patients is a different category of problem. In Huntington’s disease, the eligible population is narrow: early-manifest disease, confirmed CAG repeat expansion, and a functional status tight enough to generate a measurable Total Functional Capacity signal over 36 months without being so advanced that the patient cannot tolerate neurosurgical delivery.

It is the right endpoint for a disease with this trajectory.

Dropout in this population carries a clinical meaning that dropout in an oncology trial does not. It is the disease doing exactly what the endpoint is designed to measure.

Regulatory Pathways Shape Confirmatory Trial Design

The agency outlined alternatives including natural history comparators and single-arm designs.

uniQure’s confirmatory design chose the more rigorous randomized path, which is why the Biologics License Application moved forward. But rigorous for regulators and feasible for sites are not the same calculation.

Standard review timelines run 10 months from application acceptance. The May 1 PDUFA date tracks that clock.

But market size does not translate to site-level capacity.

The neurosurgical centers capable of delivering intrathecal or stereotactic gene therapy are a short list. Those same centers are fielding competing protocol requests from every other sponsor in the broader neurological gene therapy space.

Credentialing, pharmacy setup, and procedure training requirements have no analogue in a standard Phase III tablet study.

Future Operational Implications for Confirmatory Protocols

That means per-patient budgets must account for the full visit schedule across three years, including the administrative burden of annual informed consent form reconsent in jurisdictions that require it. It also requires coordinator time for serious adverse event narrative writing when an event occurs in a patient who is 28 months post-dose.

Serious adverse event reporting clocks run from awareness, not from dose date.

It falls off because nobody put a line item on it.

For sites accepting randomized patients into the standard-of-care arm, the operational question is equally concrete. What is the protocol amendment plan when commercial approval arrives and the standard-of-care patient wants to cross over?

The Phase I/II U.S. program did allow four control patients to cross over to treatment. If the confirmatory design includes a pre-specified crossover provision, sites need an amendment-ready Institutional Review Board submission package that can move within 30 days of approval.

If it does not include one, sites need a conversation script for the principal investigator that does not constitute protocol deviation counsel.
