The plan of the Court of Auditors to save a model of Ehpad “out of breath”

Bedtimes at 6 p.m. or even 4 p.m., dinners served at snack time, toilets that barely last seventeen minutes, overconsumption of psychotropic drugs… The current model of residential establishments for dependent elderly people (Ehpad ) East ” breathless “, estimates the Court of Auditors, in a report made public, Monday, February 28, on “medical care for the elderly” in these structures. The financial magistrates draw up the list of mistreatments resulting from the chronic lack of nursing staff and doctors. In support of this assessment, they audited fifty-seven establishments, nineteen of which were lucrative private ones.

As luck would have it, no nursing home from Orpea is included in the sample. The checks were carried out in 2020 and early 2021, i.e. before the release of Victor Castanet’s book The Gravediggers (Fayard, 22.90 euros) which denounces “the cost optimization system” of the private commercial group.

La rue Cambon argues that his photography is no less a reflection of reality since “public nursing homes are not particularly better managed than lucrative private nursing homes”, noted Pierre Moscovici in reporting the conclusions of this work, Wednesday, February 23, in the Senate. The quality of care is not related “of a public or private nature [de l’Ehpad] but to the effectiveness of the supervision linked to the triptych director-doctor-coordinating nurse”, he explained to the social affairs commission, sponsor of the report.

Despite public credits up 30% since 2011, which exceed 14 billion euros, cis increased expenses “are not enough to meet the needs of an increasingly fragile population”, notes the Court of Auditors. The growing proportion of residents with severe dependency, 54%, or suffering from cognitive disorders, 57%, requires a significant reinforcement of nursing auxiliaries, doctors, nurses, psychomotor therapists, psychologists, facilitators, etc. The first The measure to be taken to strengthen the workforce of nursing homes consists, according to the Court, of reviewing the way in which the Regional Health Agencies (ARS) allocate them allocations.

Administrative heaviness

The ARS apply an outdated needs assessment grid because it does not sufficiently take into account the time needed to prevent serious dependency and support residents with cognitive disorders. Baptized “Pathos”, this grid was overhauled by the ministry in 2015. The new version is however still not in application… Its implementation would allow an increase in the staff supervision rate for a cost which would be between 638 million and 1.2 billion euros per year. However, the Court does not specify how many additional carers would be recruited in this way.
