Understanding Glycated Albumin: A Guide to Defining Accurate Blood Sugar Control

The 33-year-old Ms. Du had an irregular schedule and seldom exercised. The result of the physical examination was that her glycated hemoglobin was as high as 10.2% (normal value <5.7%). She was worried about having diabetes and went to the endocrinology and metabolism clinic. Her fingertip blood sugar level and fasting blood sugar level were both normal. The glycated albumin test showed that the result was 11.8% (normal value <15.8%).

These conditions of glycated hemoglobin cannot be used as indicators of whether blood sugar control is good or bad.

Colin Vincent, Director of Endocrinology and Metabolism at Renai Chang Gung Memorial Union Hospital, discovered in a recent case that although the test showed that the glycated hemoglobin spiked, it was ultimately confirmed to be a false alarm and he did not suffer from diabetes. Lin Wensen said that because glycated hemoglobin cannot be used as an indicator of blood sugar control in some cases, the average blood sugar test includes glycated hemoglobin, which represents the average value of 3 months, and glycated albumin, which represents 2-4 weeks. When glycated hemoglobin is used, When the blood sugar status cannot be truly reflected (common in patients with various anemias or kidney dialysis), it is recommended to consider testing glycated albumin to provide doctors with a correct diagnosis.

Glycated albumin is suitable for these situations

Lin Wensen explained that glycated hemoglobin is the ratio of glucose in blood to hemoglobin in red blood cells. From this, the average blood sugar in the past three months can be calculated. When there is a problem with red blood cell production and metabolism or there is an abnormality in the hemoglobin structure, glycated hemoglobin cannot reflect the average blood sugar. For example, in end-stage renal disease, variant hemoglobin, and various causes of anemia, glycated albumin can be measured to evaluate blood sugar status.

Glycated albumin is the ratio of glucose in the blood to albumin in the plasma. The standard value is generally between 11% and 16%. It can be used to calculate the average blood sugar in the past 2-4 weeks and is used in short-term blood sugar monitoring of pregnant women with gestational diabetes and other causes. Those with blood diseases or anemia causing glycated hemoglobin to be unable to truly reflect blood sugar control status.

Lin Wensen pointed out that there are four ways to diagnose diabetes:

1. Normal fasting blood sugar for 8 hours should be less than 100 mg/dl (mg/dl), and the test result exceeds 126 mg/dl.

2. If diabetes is still suspected despite normal fasting blood sugar, consider doing a 75-gram glucose tolerance test. Two hours after drinking the sugar water, the blood sugar is greater than 200 mg/dl.

3. When there are obvious symptoms of hyperglycemia (dry mouth, polyuria, weight loss, etc.) and random blood sugar exceeds 200 mg/dl.

4. If there is no hemoglobin variation, the glycated hemoglobin is greater than 6.5%.

Do this every day to control blood sugar

The number of people with diabetes in Taiwan has exceeded 2.2 million, and 1 in every 9 adults has diabetes. When glycated hemoglobin is reduced by 1%, the risk of complications can be reduced by 40%, and the mortality rate from macrovascular disease can also be reduced by 25%. Vincent Lin called on high blood sugar or excessive fluctuations in blood sugar to cause oxidative stress and damage to vascular endothelial cells. Good blood sugar control can reduce the occurrence of chronic complications of diabetes, including: heart disease, stroke, amputation, blindness, and kidney dialysis. Healthy eating, regular exercise, adequate sleep and weight management are all indispensable conditions for controlling blood sugar.
