Unlike the 2018–2020 outbreak in Congo, which was driven by the more common Zaire species, this epidemic involves the rare Bundibugyo virus. Responders face a critical disadvantage: there is no licensed vaccine or proven medical treatment specifically for this strain. Most vaccines developed over the past decade targeted the Zaire variant, leaving health workers with fewer tools to halt transmission.

Surveillance Failures, Aid Cuts, and Frontline Strain

The virus gained a significant head start before the epidemic was officially declared in mid-May. Early infections were misdiagnosed—sometimes as peritonitis—and mishandled laboratory samples sent to Kinshasa allowed the pathogen to circulate undetected for months. A report published in July in the peer-reviewed journal Science concluded that the outbreak originated at least as early as January on the outskirts of the mining town of Mongbwalu.

Containment efforts continue to stumble as World Health Organization officials estimate that roughly 80% of new infections emerge outside known transmission chains. Contact tracers in Ituri Province are overwhelmed by caseloads, and surveillance teams lack the necessary resources to keep pace.

Meanwhile, clinical trials are underway.

Enrollment has begun for clinical trials evaluating two post-exposure prophylaxis drugs in Ituri Province, alongside separate vaccine trials hosted in the United Kingdom and Canada. Whether these experimental measures can outpace the fastest-growing Ebola crisis on record remains to be seen as leaders from the WHO and the Africa Centres for Disease Control and Prevention coordinate their emergency response in Kinshasa and Bunia.
